If you do not find a list of application forms matching your
equipment, you may download G100, HRA 100 and
G101 separately. After you have submitted G100 and HRA 100, the AQMD
will notify you if additional information is required for your equipment
type.

The SMAQMD cannot accept permit applications via the Internet.
Submit the following, either by mail or in person to SMAQMD, 777 12th
Street, 3rd Floor, Sacramento, CA 95814:
Each equipment unit requires a separate application for
a Permit to Operate.
A permit fee will be assessed for each new or modified source based on
the type and size of the source. Fees are listed in the instructions for
each equipment type. Every applicant for a Permit to Operate shall pay
one half the estimated initial permit fee
upon filing of the application, and the remainder upon issuance of the
permit.
If you operate equipment without a Permit to Operate,
you are subject to legal action, including civil or criminal penalties
up to $50,000 per day of operation or up to six months in jail. For additional
