Customer Service Survey

If you've received funding from one of the Sac Metro Air District grant programs, we're asking you to participate in a short customer service survey. The survey addresses all aspects of the grant program from application submission through your receipt of the grant payment. Tell us what we're doing well, and let us know where we need to improve . Need to substitute and finish your grant contract with different equipment?

We know that sometimes things happen that are out of your control. If your equipment or vehicle had an accident or is no longer operating, but it is still under contract, you have options. Complete the Equipment Substitution Request Form and submit to myusage@airquality.org. Click the button below to access the form.

Need to transfer your equipment under contract?

If you need to transfer ownership of your equipment while it is under contract in one of our grant programs, contact us as soon as possible by completing the Equipment Transfer to New Owner Request Form and submit to myusage@airquality.org. Click the button below to access the form.
